POSITION SUMMARY/JOB PURPOSE:

The Clinic Nurse assists with meeting the needs of complex medical patients, including assessment, education, referrals, and follow-up care for individuals with HIV in the Basin Assistance Services (BAS) program. This position provides nursing care including but not limited to administering medications, immunization and collecting specimen including blood and urine and treatments in compliance with a medical provider's orders, BAS policies, and State of Texas guidelines. The Clinic Nurse interprets information obtained from patient and others, including the health care team, and documents all clinical care in the patient's electronic medical records accordingly. This position provides general care coordination including but not limited to patient triage, phone call-backs, prescription refills, lab ordering and interpretation, prior authorization of medication when required. 

This position works independently, under limited supervision, reporting major activities through periodic meetings.

EDUCATION, EXPERIENCE, OTHER QUALIFICATIONS:

Education Required:  High School Diploma or GED. 

Experience Required:  At least 1 year experience in medical setting or nursing field.

Registration, Certification, Licensure or other Qualifications Required: 

  • Must maintain a valid Texas Driver's license, auto liability insurance and a driving record acceptable to PermiaCare's insurance requirements.
  • Required to pass criminal history and background checks as well as pre-employment
  • drug screen.
  • Current and valid Texas certification or license as Medical Assistant, Certified Nursing Assistant, Licensed Vocational Nurse, or Registered Nurse.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Schedule clients to be seen in the clinic as needed.
  • Triage with medical services in clinic as indicated by a medical provider.
  • Administer immunizations as required by provider and state standards.
  • Review client chart and immunization schedule requirements to determine needed immunizations and administer during clinic visit or schedule to be administered via use of standing orders.
  • Review client charts after each clinic and submit orders for lab work requested by each provider.
  • Collect specimens as ordered by medical providers or standing orders.
  • Review client charts for pending orders or need of lab work to be seen by medical provider.
  • Review and refill medications as appropriate.
  • Maintain medical records in all EHRs and databases.
  • Maintain BAS inventory and adequately stock medical supplies.
  • Maintain medical scanning.
  • Keep proper inventory of vaccines.
  • Monitor refrigerator and freezer temperature daily.
  • Ensure clinic area is sanitized properly after each use.
  • Coordinate referrals given to case managers.
  • Enter accurate and appropriate documentation of services within timeframe required.
  • Maintain confidentiality of sensitive records and treatment information, client files and protected health information in compliance with HIPAA, laws, rules and regulations, and established procedures.
  • Maintain regular and reliable physical on-site attendance. Regular attendance, dependability, and promptness are required for the scheduled work day 100% of the time, to ensure consistency and completeness of program's processes.
  • Comply with the Abuse, Neglect, and Exploitation policy and reporting requirements.
  • Adhere to the Code of Conduct and Standards of Behavior policy requirements.
  • Establish and maintain effective work relationships with individuals served and their families, supervisors, co-workers and visitors by demonstrating cooperative, courteous and respectful behavior at all times.
  • Communicate regularly with supervisor.
  • Open and process mail/email in a timely manner.
  • Answer phone, collect phone messages and respond to requests timely and accurately.
  • Maintain safe and clean working environment by complying with procedures, rules and regulations.

  • Display professionalism when representing PermiaCare and the program in the community.
  • Maintain compliance with legal requirements and company policies and procedures.
  • Maintain valid and current driver's license, auto insurance, acceptable driving record and reliable transportation at all times. Driving may be required for this position.
  • Complete all training as assigned prior to due date.
